A registration application cannot be considered complete until all the elements required have been submitted to the school:

Registration Fee of $125 per student (non-refundable)
Tuition Contract Form (one per family)
Preschool Form (if applying for Preschool)
2-Year-Old Form (if applying for 2-Year-Old Program)
Volunteer Interest Form (optional)

Copy of Birth Certificate
Copy of Baptismal Certificate (for Catholic students only)
Registration Fee of $125 per student (non-refundable)
Tuition Contract (one per family)
New Student Information form (one per student)
Preschool Form (if applying for Preschool)
2-Year-Old Form (if applying for 2-Year-Old Program)
Volunteer Interest Form (optional)
Illinois Certificate of Health
Illinois Dental Health(for those entering K, 2, and 6)

Applications will not be considered until all required elements are received by the school office. Mail your completed application and registration forms, along with the application fee, to:
