Health Records

All preschool, kindergarten, fifth grade and newly enrolled students must have an up-to-date current physical examination and all immunizations required by the Illinois School Code. This health record must be submitted prior to the first day of school and becomes part of the student's file. Physical examination forms may be secured from the school office.
